Medicaid/CHIP Perinatal Application Conditions and Preparation Documents

Please prepare the following documents:
1. Social Security cards for the whole family

2. US passport

3. Birth certificate, citizenship certificate, green card (1-551)

4. Proof of address : (Proof of address can be any of the following)
Texas driver's license or green card water, electricity, and other bills, bank statements, leases or home purchase agreements

5. Proof of income of the whole family in the last 90 days: (Proof of income can be any of the following)
Tax return W2 or 1099 for the most recent year. If you live on savings, please provide the most recent month's bank statement. If you rely on others to fund, you need to provide a letter of proof from the sponsor (Guangyan Society has a template)
Letter of certification signed by the employer

6. Pregnancy certificate (a doctor's certificate is required)
